Older adults benefit by senior support service available at Norman P. Murray Community and Senior Center

Mission Viejo seniors are taking advantage of a beneficial service designed to help them maintain healthy independence and well being.

At the City’s Norman P. Murray Community and Senior Center, seniors – facing difficult life circumstances – can opt for case management. The program, offered through Age Well Senior Services, provides assistance for families and their senior loved ones in finding resources, making decisions and managing stress. With an appointment at the community center, case management essentially provides “one-stop shopping information” for seniors and family members to gain access to available resources throughout the country.

The program is facilitated by case manager Lauren Freeman. In her role, Lauren visits the person’s home and assesses his or her functional skills, living situation, cognitive status, unmet needs, strengths and knowledge of community resources to develop and coordinate a plan of care. She works with other health care professionals and uses community resources to best meet that individual’s needs. She helps with referrals for meals on wheels; adult day services; assisted living or board and care placements; senior center activities; behavioral health referrals; Medicare and medical information; physician and elder law referrals; transportation information; medical insurance counseling; in-home help and support group meetings; and more.

Lauren can also help people with decisions connected to aging.
